Job Overview:
We are seeking an experienced and compassionate Practice Nurse to join our dynamic primary care team at Mill Hill Surgery. As a key member of our multidisciplinary team, you will provide high-quality nursing care to our patients, promoting health and wellbeing in the community. The ideal candidate will have a broad range of clinical skills, excellent communication abilities, and a strong commitment to patient-centered care.
Our practice, Mill Hill Surgery in Ealing / Acton, is composed of six (6) GP partners, along with a Nursing team including HCA support, a newly appointed GPA role, and a comprehensive admin support team providing primary care services to a registered population of approximately 7000 patients across the borough of Ealing.

As a GP Training Practice, we offer excellent support, supervision and training to support ongoing professional development.
Key Responsibilities:
As a Practice Nurse, your duties will include, but are not limited to:
1. General Nursing Duties:
o Provide general nursing care for patients in the practice, ensuring a high standard of clinical practice.
o Assess, plan, implement, and evaluate patient care in line with clinical guidelines and best practices.
o Carry out basic wound care, dressings, and management of minor injuries.
o Provide health education and advice, promoting self-care where appropriate.
2. Chronic Disease Management:
o Provide care and support to patients with long-term conditions such as asthma, COPD, diabetes, hypertension, and cardiovascular diseases.
o Conduct routine assessments, monitoring, and management plans for patients with chronic illnesses.
o Offer lifestyle advice and coordinate care in collaboration with other healthcare professionals.
3. Immunisations and Vaccinations:
o Administer childhood and adult immunisations as part of the vaccination schedule.
o Provide travel vaccinations and advice on travel health.
o Participate in seasonal vaccination campaigns (e.g., flu clinics).
4. Health Screening and Assessments:
o Carry out health checks for patients, including blood pressure monitoring, phlebotomy, and weight management.
o Conduct cervical smears and sexual health screening.
5. Minor Illness and Injury Management:
o Offer wound care and first aid, including assessment of minor injuries and referral when appropriate.
6. Medication Management:
o Ensure accurate documentation.
o Monitor patients on specific medications, such as anticoagulants or hormone injections, and arrange for regular reviews.
7. Patient Education:
o Educate patients on disease prevention, healthy living, and self-management of conditions.
o Provide emotional support and guidance for patients with chronic illness or other complex health needs.
8. Administrative Duties:
o Maintain accurate, up-to-date patient records, in line with legal and confidentiality requirements.
o Coordinate patient appointments, ensure efficient patient flow, and contribute to the smooth running of the practice.
o Participate in audit and quality improvement initiatives to enhance patient care and practice performance.
9. Multi-disciplinary Team Collaboration:
o Work closely with GPs, healthcare assistants, and other members of the healthcare team to deliver integrated patient care.
o Participate in clinical meetings and support the implementation of practice-wide policies and procedures.
o Provide support and mentorship to other staff members as needed.

Qualifications & Experience:
• Registered Nurse with valid NMC registration.
• Minimum of 1 years of clinical experience in a general practice, primary care.
• Experience in chronic disease management, immunisations, and general nursing duties.
• Qualification in cytology and immunisations.
• Additional qualifications in areas such as diabetes management, asthma care, or travel health are desirable.
