Job description
We have an exciting opportunity for an nurse to manage a caseload of our housebound patients. This is a dynamic and autonomous role that allows the right candidate to manage their own caseload. The role incorporates managing the long term conditions of patients who are housebound and dealing bringing a holistic approach to managing their needs

We are a large group of 9 practices in north Southwark with a large nursing team of over 20 practice nurses, community nurses, nurse associates and health care assistants all supporting each other and with established team working.

We also have a great variety of other health care professionals such as paramedics, Gps and pharmacists

All our practices have a long history of supporting training for all health care professionals and there are many opportunities to develop your career and the role

Experience

Essential

Minimum of 2 years post graduate experience in a community or primary care setting
Working within a multi-disciplinary team
Experience of managing a case load, implementing care pathways, protocols and clinical guidelines.
Experience of management of complex long-term conditions and can demonstrate the importance of chronic disease management and rehabilitation in the community.
Desirable

Worked as community or district nurse
Minimum of 2 years post graduate experience
Qualifications

Essential

Registered Nurse
Current NMC Qualification
Post registration courses in chronic disease management
Desirable

Independent non-medical prescriber (V300)
Primary care experience
Working towards or interest in working towards advanced nurse practitioner competencies
Nursing Degree Qualification BSc or MSc or working towards
Job Type: Full-time

Pay: £45,000.00-£48,800.00 per year
