Health and Wellbeing Coach

The purpose of the role is to develop collaborative relationships and work in partnership with health, social care, and community and voluntary sector providers and multi-disciplinary teams to holistically support patients wider health and well-being, public health, and contributing to the reduction of health inequalities; provide education and specialist expertise to PCN staff, supporting them to improve their skills and understanding of personalised care, behavioural approaches and ensuring consistency in the follow up of peoples goals with MDT input; work with the GPs and other primary care professionals within the PCN to identify and manage a caseload of patients, and where required and as appropriate, refer people back to other health professionals within the PCN, in accordance with the needs, priorities and any urgent support required by individuals.

Main duties of the job

– Provide education and specialist expertise to PCN staff, supporting them to improve their skills and understanding of personalised care, behavioural approaches and ensuring consistency in the follow up of peoples goals with MDT input.
– Work with the GPs and other primary care professionals within the PCN to identify and manage a caseload of patients, and where required and as appropriate, refer people back to other health professionals within the PCN, in accordance with the needs, priorities and any urgent support required by individuals
– To be the first point of contact at GP practices where coaching services will be delivered, and to receive and manage referrals. Using existing IT and MDT channels to screen patients, with an aim to identify those that would benefit most from health coaching
– To provide personalised support to individuals, their families, and carers to support them to be active participants in their own healthcare; empowering them to manage their own health and wellbeing and live independently.

About us
Sutton has a population of approximately 200000 residents registered to 23 practices and there are currently 4 Primary Care Networks each serving a population of approximately 50000 patients.

Sutton is looking to employ a Health and Wellbeing Coach under the additional roles reimbursement scheme (ARRS). They will be accountable to the Primary Care Networks, working in individual PCNs and most likely allocated to specific practices within the PCN (sometimes more than one) to work closely with and support that practice team. These individuals may also work across more than one PCN in some cases where flexibility allows and for the purposes of cross cover and to ensure general practice resilience in Sutton.

– Work with the GPs and other primary care professionals within the PCN to identify and manage a caseload of patients, and where required and as appropriate, refer people back to other health professionals within the PCN, in accordance with the needs, priorities and any urgent support required by individuals
– To be the first point of contact at GP practices where coaching services will be delivered, and to receive and manage referrals. Using existing IT and MDT channels to screen patients, with an aim to identify those that would benefit most from health coaching
– To provide personalised support to individuals, their families, and carers to support them to be active participants in their own healthcare; empowering them to manage their own health and wellbeing and live independently through:
– Coaching and motivating patients through multiple sessions to identify their needs, set goals, and supporting patients to achieve their personalised health and care plan objectives
– Providing interventions such as self-management education and peer support
– Supporting patients to establish and attain goals that are important to the patient
– Supporting personal choice and positive risk taking while ensuring that patients understand the accountability of their own actions and decisions, thus encouraging the proactive prevention of further illnesses
– Working in partnership with the social prescribing service to connect patients to community-based activities which support them to take increased control of their health and wellbeing; increasing patient motivation to self-manage and adopt healthy behaviours
-To work with patients to help them understand their level of knowledge, skills and confidence when engaging with their health and well-being and subsequently supporting them in shared decision-making conversations.
-Utilise health coaching skills to support people with lower levels of activation to develop the knowledge, skills, and confidence to manage page their health and wellbeing, whilst increasing their ability to access and utilise community support offers.
-To work as part of a Multi-Disciplinary Team, participating in progress meetings, ensuring that the MDT is kept fully up-to-date with progress reports.
-To ensure that all complaints and compliments received are recorded in line with organisational policy.
-To recognise the importance of safeguarding procedures within the project, and ensuring that any concerns, disclosures or allegations of abuse are immediately and correctly reported.
-To embed equality, diversity and inclusion best practice into all aspects of work.
-To work to the requirements of the organisations quality standards, and other service-specific quality accreditations as required, abiding by organisational ethos and principles
-To attend and participate in regular supervision, appraisals, training and other internal meetings.

Person Specification

Qualifications – Essential
-Enrolled in, undertaking or qualified from appropriate health coaching training covering topics outlined in the NHS England and NHS Improvement Implementation and Quality Summary Guide, with the training delivered by a training organisation listed by the Personalised Care Institute

Knowledge and Skills – Essential
-Knowledge of person-centred approaches;
-Understanding of approaches to working with people with mental health needs;
-Knowledge of Equal Opportunities, Diversity & Inclusion policies and practice;
-Knowledge of Safeguarding Vulnerable Adults policies and procedures;
-An open mind and creative approach to working with people;
-Ability to relate to local people with positive and non-judgemental approach;
-Ability to organise own time, demonstrating flexibility and initiative in approach;
-Ability to work effectively with colleagues, patients, external organisations and partners;
-Effective communicator, both verbally and in writing;
-Ability to assess risks, anticipate difficulties and successfully address them.

Knowledge and Skills – Desirable
-Ability to work across multiple sites within Sutton;
-Willing to work flexibly within a team to cover annual leave or sickness, and to provide occasional cover for evenings and weekends;

-Use of GP IT Systems (preferably EMIS Web and DOCMAN).

Experience – Essential
-Experience working in health, community or voluntary sector;
-Experience of working with vulnerable adults including people with mental health problems;
-Experience of working with people facing a range of barriers and social issues;
-Experience of working with individuals (1-2-1) and groups of people in different settings to help them achieve their goals;
-Ability to engage a wide range of different people and to inspire them to make their own decision and take their own actions;
-Experience of working in multi-disciplinary teams.
