JOB SUMMARY
We are looking for paramedics who are keen to develop their skills, to join the Nexus team at one of our 9 practice sites in north Southwark. Paramedics will work in one of our multidisciplinary primary care teams running clinics in the surgery and home visits to our housebound patients. Our patient population is varied reflecting the diverse communities of Southwark and inner London with a mix of students, young families, different ethnic groups, asylum seekers and professionals. All our practices are training practices with experience in training a wide mix of practitioners.
MAIN DUTIES OF THE JOB
Whether newly qualified or highly experienced, you will be exposed to the broad scope of general practice via assessment of patients in the surgery and in their own homes. Our paramedics triage patients over the phone, provide face to face assessment in the practice or patient’s home, and use their diagnostic skills on a daily basis to help put together management plans under the supervision of our experienced GP team.
You should be a registered paramedic who has the ability and confidence to safely and comprehensively perform full systems assessments and, with senior colleague input, agree and implement a plan of care.
Your working week
Morning clinic in surgery with GP supervision seeing patients with acute medical problems
Afternoon home visits for housebound patients with acute medical problems for assessment or follow up. Review of those with long term conditions unable to get to the surgery.
Liaison and referral to other primary health care services
Weekly site based clinical meeting to discuss clinical cases, learning events and organisational developments.
Tutorial with supervisor

MAIN DUTIES
Home visits – assess and treat acutely unwell patients in their home, making any necessary referrals to other acute community services and/or multidisciplinary teams of the Organisation
In the surgery- assess and treat patients presenting with acute illness undertaking face to face or telephone consultations as required.
Undertake telephone triage as required
Liaise with and refer to community and secondary care providers as needed
Arrange urgent admission for acutely unwell patients
Ensure clinical practice is safe and effective and remains within boundaries of competence, and to acknowledge limitations.
Advise patients on general health care and minor ailments, with referral to other members of the primary and secondary health care team as necessary.
Works from the Nexus site and within communities as an autonomous practitioner caring for patients and families.
Works as an autonomous practitioner, in accordance with the Health Professions Council. Ensure that personal and professional clinical standards are maintained.
ADMINISTRATION AND PROFESSIONAL RESPONSIBILITIES
Participate in the administrative and professional responsibilities of the practice team.
Ensure accurate and legible notes of all consultations and treatments are recorded in the patient’s notes.
Ensure the clinical computer system is kept up to date, with accurate details recorded and amended
Ensure accurate completion of all necessary documentation associated with patient health care and registration with the practice.
Ensure collection and maintenance of statistical information required for regular and ad hoc reports and audit.
Attend and participate in practice meetings as required.
Restocking and maintenance of clinical home visit bag.
Person Specification:
ESSENTIAL
Qualified Advanced Paramedic
Registered with the HCPC
Evidence of appropriate continuing professional development to maintain up to date knowledge and on- going competencies in all aspects of the role.
A minimum of 2 years experience
Self- directed practitioner
Commitment to provide effective and high quality service.
Ability to work within a multidisciplinary team and across boundaries.
Maintaining good medical practice and further developing competence and performance
Ability to work flexibly to meet service needs.
Commitment to working in the NHS and improving care given to patients.
