For use when a practice has not received details of an outpatient appointment or a discharge summary within the required time frames.

This resource forms part of our safe working pack.

The NHS Standard Contract specifies that practices should receive discharge letters this is no later than 24 hours following discharge and outpatient letters within seven days of an appointment. Failing to meet these timelines wastes resources in general practice and distresses patients who expect to be able to discuss their care with their GP.
