There is no requirement for them to fill out time consuming and widely varying trust referral proformas, this template supports GPs in pushing back against this.

GPs are required to make referrals that are:

  • clinically appropriate,
  • detail the required information to ensure a safe transfer of care to the specialist, and
  • that are in line with both the NHS Standard Contract 2024/25 Service Conditions: Acceptance and Rejection of Referrals 6.13 Subject to SC6.3 and to SC7 (Withholding and/or Discontinuation of Service), and the Primary Medical Services Contract.
